This template defines a structured, outcome-driven Agile transformation roadmap.

The roadmap should be capability-based, not tool-based.


1. Transformation Vision

1.1 Desired Future State

Describe the intended operating model in 12 to 24 months.

Cover:

  • Delivery model (product vs project)
  • Governance model
  • Funding model
  • Coaching model
  • Metrics model
  • Cultural attributes

1.2 Measurable Outcomes

Define 3 to 7 transformation outcomes.

Examples:

  • Lead time reduced by 30 percent
  • Predictability variance under 15 percent
  • Customer validation integrated into every release cycle
  • Cross-functional teams fully accountable for delivery

2. Current State Assessment

2.1 Delivery Health

2.2 Cultural Health

3. Transformation Phases

Phase 1: Stabilize

Goal: Create baseline delivery control and visibility.

Exit criteria:

  • Incentive realignment

Exit criteria:

  • Reduced cross-team wait time
  • Clear portfolio decision rights
  • Consistent operating model across teams

Phase 4: Sustain

Goal: Ensure transformation durability.

Focus areas:

  • Coach exit transitions
  • Internal champions enabled
  • Periodic health assessments
  • Continuous improvement culture embedded

Exit criteria:

  • Teams self-diagnose bottlenecks
  • Improvement backlog active without external prompting
  • Leadership behavior aligned with Agile principles

4. Governance Model

Define:

  • Decision rights
  • Escalation pathway
  • Funding model alignment
  • Risk management approach
  • Review cadence (monthly, quarterly)

5. Metrics Dashboard Alignment

Define dashboard categories:

  • Flow metrics
  • Quality metrics
  • Delivery predictability
  • Product value signals
  • Cultural health indicators

Each metric must include: - Definition - Formula - Target range - Anti-pattern warning
