Quarterly Executive Transformation Review

Audience: - CIO - CTO - VP Engineering - Product leadership - Transformation sponsors

Purpose: Provide structured review of transformation health, systemic impediments, and strategic alignment.

Timebox: 90 minutes


1. Executive Summary (10 minutes)

Provide a concise overview:

  • Overall maturity trend
  • Delivery stability trend
  • Major systemic risks
  • Major wins

  • Quality trend: Improving / Stable / Declining

2. Flow and Delivery Review (20 minutes)

Review:

  • Lead time trend (quarter over quarter)
  • Throughput stability
  • Commitment reliability
  • Blocked work percentage

Discussion prompts:

  • Where is variability increasing?
  • Where is WIP accumulating?
  • Are cross-team dependencies slowing flow?

Decision outputs:

  • Structural changes required
  • Resource adjustments
  • Dependency ownership clarification

3. Product and Outcome Review (15 minutes)

Review:

  • OKR progress
  • Feature adoption data
  • Time from release to feedback

Discussion prompts:

  • Are we shipping validated value?
  • Where are we delivering output without outcome?
  • Are we funding discovery sufficiently?

Decision outputs:

  • Portfolio reprioritization
  • Discovery investment changes
  • Outcome measurement improvements

4. Quality and Technical Health (15 minutes)

Review:

  • Defect escape rate
  • Rework rate
  • Deployment frequency
  • Technical debt indicators

Discussion prompts:

  • Is quality debt accumulating?
  • Are deployment constraints slowing innovation?
  • Are we under-investing in engineering excellence?

Decision outputs:

  • Investment adjustments
  • Quality threshold changes
  • Technical modernization priorities

5. Cultural and Governance Health (15 minutes)

Review:

  • Psychological safety trend
  • Retro action completion rate
  • Escalation frequency
  • Escalation resolution latency

Discussion prompts:

  • Are teams surfacing risk early?
  • Are leaders removing impediments quickly?
  • Is governance slowing flow?

Decision outputs:

  • Incentive alignment changes
  • Leadership behavioral adjustments
  • Escalation model refinements

6. Maturity Review (10 minutes)

Review maturity scoring worksheet:

  • Lowest scoring dimension
  • Most improved dimension
  • Systemic constraint limiting progress

Decision outputs:

  • Next quarter transformation focus
  • Coaching allocation
  • Governance adjustments

7. Executive Commitments (5 minutes)

Each executive states:

  • One structural change they will sponsor
  • One behavior they will model
  • One systemic impediment they will remove

Document commitments publicly.


8. Review Outputs

Record:

  • Risk mitigation plans
  • Coaching focus updates

Publish summary within one week.

Transformation progress must be visible at the executive level.
