Purpose: Provide a structured scoring mechanism to accompany the Agile Maturity Model.
This worksheet supports:
- Quarterly team assessments
- Program-level capability reviews
- Transformation roadmap alignment
- Coaching backlog prioritization
Scoring scale:
1 = Ad Hoc
2 = Structured
3 = Predictable
4 = Optimized
5 = Adaptive
Score based on observable behavior and metrics, not opinion.

Weighted Maturity Score:
7. Interpretation Guide¶
1.0 to 1.9 = Ad Hoc
2.0 to 2.9 = Structured
3.0 to 3.9 = Predictable
4.0 to 4.5 = Optimized
4.6 to 5.0 = Adaptive
Do not chase a level. Focus on removing constraints that limit flow and learning.
